Enrollment Forms

Enrollment forms collect information every time a student registers or enrolls — permission slips, medical details, liability waivers, or teacher questionnaires. Attach a form to a term, class, or program, and families must complete it during enrollment before they can check out.

Enrollment forms are family forms. Only family forms can be attached to terms, programs, classes, events, and applications — web forms can't.

The student application covers brand-new applicants only; enrollment forms are how you collect information from everyone who enrolls, including returning students. See Customizing the Student Application for application forms.

Require a form on a term, class, or program

First build the form under Enrollment → Family forms — see Creating Custom Forms. Then:

  1. Go to Enrollment and open the term, class, or program.
  2. Find the Enrollment Forms section — on the term's page, or on the class or program's Details tab.
  3. Click Add.
  4. Under Select a form, pick the form and click Add. Only family forms are listed.

Repeat to require several forms — click Reorder forms and drag them to set the order families fill them in, then Finish reordering forms. Use the Remove action on a row to stop requiring a form (confirm with Remove); responses already collected are kept.

Whenever a student enrolls, they're asked to complete all required forms before checkout. Forms attached to the term apply to every registration; forms on a class or program apply only to that enrollment.

Note: Completed responses appear on the student's or parent's profile under Enrollment → Families — see Form Responses.

Request a form from one specific student

You can also send a form to a single student's family instead of requiring it of everyone:

  1. Open the class or program, go to its Students tab, and click the student to open their profile.
  2. In the Forms section, click New Form Request.
  3. Choose the Form. Leave Notify the contact about this form request checked to let the family know, or uncheck it to add the request quietly.
  4. Click Create.

The form appears in the student's Forms list with a status you can track, and the family completes it from their portal. Click a response row to view what was submitted.
