These are the field types available when you click Add field in the form builder. Every input field has a label, an optional description shown under it, and a Make required option.
Text
Displays formatted text on the form — it doesn't collect an answer. Use it for instructions, section introductions, or legal and disclaimer wording people must read. Supports rich formatting like bold text, links, and lists.
Short answer
A single-line text box. Best for short responses like a name, an email address, or an emergency contact number.
Long answer
A multi-line text box for longer, free-form responses — explanations, feedback, or anything that needs a sentence or more.
Multiple choice
Presents a list of choices and lets the person pick exactly one. When filling out the form, the choices appear as a searchable dropdown. Add as many choices as you need in the Choices list.
Checkbox list
Also presents a list of choices, but the person can select all that apply.
Date picker
Lets the person pick a date from a calendar. Check Include time if you also need a time of day.
File upload
Lets the person attach a file — a signed document, an immunization record, a photo. Uploaded files are stored privately; you can download them when viewing the response, and they're also saved to the contact's Files tab.
